A manager escalates to their executive; only the executive escalates to the operator.
The record
The executive layer exists to absorb decisions, and a direct channel from six managers to the operator collapses it. A manager blocked on something outside their remit raises it to the executive named on their seat. That executive either decides or escalates with a recommendation attached — an escalation arriving without one gets sent back.
