A confidently wrong memory does more damage than no memory at all

An agent with no record asks. An agent with a stale record acts, and acts wrongly.

VerifiedConfirmed against the source by a named reviewer.

The record

The failure mode is asymmetric and that asymmetry drives the whole retention policy. Missing information produces a question, which is cheap. Wrong information produces a confident action, which is not. Anything time-sensitive carries a review date, anything contradicted gets corrected or deleted the same day, and deletion is always preferred to hedging — a memory qualified into uselessness still ranks in results.

No vector store is connected, so this record has no embedding. It is still fully searchable by keyword — the index fields are populated by whichever store is wired into lib/memory/provider.ts.
